You must present your card at every visit!
Please be aware that filing an insurance claim does not guarantee payment of that claim by your
insurance carrier.
If we do participate with your health plan, we will be happy to file a claim with your
insurance carrier for services rendered. Please fam- iliarize yourself with your insurance benefits
prior to your appointment to minimize any confusion regarding your plan coverage. A co-payment or
deductible may be due at the time of service. Please consult your health plan to determine these
benefits.
If we do not participate with your health plan, payment for services rendered will be
collected in full at the time of service. As a courtesy, we may file a claim with most insurance
plans.
If you feel that your claim has not been handled correctly by your insurance carrier, please contact
your plan as soon as possible regarding any discrepancies. Please refer to the number listed on your
insurance card.
